I regularly travel to all of the purple areas on this map. All In-Home Consults are subject to a $50 Home Visit Convenience Fee. You can avoid this fee by booking an Office Consult.

For families in the grey shaded towns on this map, there will be an additional $50 Extended Travel Fee that is ineligible for insurance reimbursement/coverage.
